Claims should be reported as soon as possible after the loss. At a minimum, please include:
01. Policyholder contact information – name, phone Number, Email Address
02. Agent contact information – name, phone number, email address
03. Detailed description of the loss – date of incident, location of incident, description of damages and injuries
You may also download the Mach 2 Incident Report form, complete it and submit it in accordance with the instructions on the form.
An adjuster will be assigned to the claim and, after reviewing the information provided, will make personal contact via the provided information.
